Internal mammary artery conduit: an additional surgical procedure in difficult coarctation of the aorta.

چکیده
Between October 1986 and February 1987 two young men with coarctation of the aorta and a hypoplastic isthmus were treated by a combined resection and isthmusplasty with implantation of the left internal mammary artery to the distal aorta. An appreciable drop in the systolic gradient was seen in both patients after the mammary artery anastomosis and haemodynamic variables across the repair continued to improve postoperatively. Digital transvenous subtraction angiography five months after operation showed a satisfactory mammary artery conduit across the repaired coarctation.

Internal mammary artery dilatation in a patient with aortic coarctation, aortic stenosis, and coronary disease. Case report
The ideal surgical approach is unclear in adult patients with coarctation of the aorta that is associated with other cardiovascular pathologies that require intervention. Standard median sternotomy allows simultaneous, coronary revascularization surgery, valve replacement and repair of aortic coarctation. However the collateral circulation and the anatomy of the mammary arteries must be determi...
متن کاملAnatomical Study of Subclavian Artery and Its Branches
Background: The branches of right and left subclavian arteries are important in conducting of blood to spinal cord, posterior cranial fossa, base of the neck, thyroid gland, wall of the thorax and abdomen. The routine use of internal thoracic artery as a conduit in coronary artery bypass grafting surgery requires appreciation of the anatomical variations of these vessels. So far, there is not a...
متن کاملClinical alternative bypass conduits and methods for surgical coronary revascularization.
Poor quality or inadequate length of venous and mammary conduits, or both, a severely calcified or atherosclerotic aorta, or diffuse coronary atherosclerosis are situations cardiovascular surgeons will be facing with increasing frequency. These conditions are more common to the increasing number of patients requiring reoperation for advancing disease and to the growing number of older patients ...
متن کاملBeta-adrenoceptor-mediated responsiveness of human internal mammary artery
The internal mammary artery (IMA) is currently the preferred conduit for myocardial revascularization. However, pre-operative vasospasm and a hypoperfusion state during maximal exercise may limit its use as a bypass graft. The mechanism of spasm has not been clearly defined. Since β-adrenoceptor activation plays a major role in vasorelaxation, the present study was carried out to investigate th...
متن کاملIntravascular Treatment of Left Subclavian Artery Aneurysm Coexisting with Aortic Coarctation in an Adult Patient
Coexistence of aortic coarctation with aneurysm of subclavian artery is a uncommon situation and may require unusual treatment in patients. A 40-year-old patient diagnosed incidentally with left subclavian artery aneurysm coexisting with aortic coarctation. Patient was initially referred for hybrid treatment. Initially ostium of the left subclavian artery was covered with a stent-graft. Over a ...
